WREB automatically sends group exam score reports on all candidates to our member state boards; however, in some instances, you are ALSO required to send proof of your exam score(s). Some of the WREB member states require the same item in different forms. Prior to submitting your request for scores, please be sure to contact the state board in the state where you wish to practice, to better determine what you will need.
- Once your scores are available, you will receive an email from WREB with a link for viewing your scores. Please DO NOT request scores until your results have been posted to your online profile.
- Requests are processed in the order they are received. The turn-around time for requests can be up to two weeks, depending on the volume of the requests.
CURRENT SCORE REQUEST PROCESSING TIME IS 10 BUSINESS DAYS.

Requests can only be made by the person who took the exam. If you are a 3rd party requesting information about someone’s score, you must have them complete this form.
Enter information as it appears on the credit card, with no spaces and no dashes in the card number.
Note: Once you have submitted your credit card information, you have entered into a financial agreement with WREB.
Sealed Success Report
SUITABLE FOR MOST LICENSURE APPLICATIONS. Report lists exam(s) attempted, dates and locations, candidate name and address as well as the pass outcome in each section, if applicable. Report includes WREB-embossed seal if mailed.
Individual Performance Report – Includes detailed score information
Not available for exams given prior to 2004.
Report includes breaks down of score received for each procedure per section as applicable. Report available for Dental, Dental Hygiene Patient-based, and Restorative candidates only.
Dental Examination History
For applicants seeking licensure by residency in California. This information is sent directly to the Dental Board of California from the WREB office.

NOT FOR LICENSURE APPLICATION. Certificate (8 x 5) suitable for framing. Includes Candidate name, location and year in which exam was successfully attempted; numerical scoring information not included.
Dental Hygiene Summary Profile Sheet
Summary sheet of validated tissue trauma, calculus errors, and probe and recession error of an unsuccessful Patient-based exam attempt.
