Request Scoring Information/Reports
WREB receives numerous requests for scores to be sent to various state dental boards. All state dental boards require different items.
Prior to sending your request for scores, please be sure to contact the state board in the state where you wish to practice, to better determine what you will need.
Some of the WREB member states require the same item in different forms; WREB has compiled a listing to see if you will need to use our services.
| Duplicate Success Cards (only available for exams prior to 2010) |
You would need to submit a request for a duplicate success card for any exams prior to 2010. |
| Original Success Reports (for exams beginning 2010) |
WREB will post these to our website once you have completed and passed your exam. You will have access to these with your user name and password. You may print your report and keep it for your records. You may also print a copy for state board submittal. |
| Sealed Success Reports (for exams beginning 2010) |
Most WREB member states will be satisfied with the copy you provide them. However, IF they require a Success Report with a WREB seal, mailed directly to them, you would submit a request for a Sealed Success Report.
As of May 2010, Texas State Board requires this report to be mailed to the candidate so that it can be included with their licensure application paperwork. |

| Notarized Photocopies |
You do not have to contact WREB. Make a photocopy of your success card/report, get it notarized and send to the state board along with any other required documents. |
In order to help us provide the appropriate documentation, please assist us by specifying what is needed. WREB automatically sends group reports on all candidates to our member state boards; however, you MAY ALSO be required to send proof of your exam score(s). If you are applying for licensure in a non-member state, you will need to use our services. Please use a separate form for each request and return with appropriate fees.

Detailed Scores
What it is: This letter provides the breakdown of scores received in each section of your Dental or Dental/Hygiene exam, with a WREB raised seal.
Beginning in 2009, with conjunctive scoring, only pass/fail results will be reported.

Individual Performance Report (not available for exams given prior to 2004)
What it is: This document breaks down the score received for each procedure, in each section of the exam. Unsuccessful candidates automatically receive this review with their result letters.

Success Card
What it is: This is a replica of the success card we previously used prior to 2010. The card lists the location of the exam, your name and address, candidate number and it shows pass outcome. It will be stamped as "Duplicate" and have the WREB raised seal.

Sealed Success Report
What it is: This is the report that WREB currently uses for exams starting in 2010. This report is the same report that is available to candidates online. It lists the exam(s) taken, the dates and locations, the candidates name and address as well as the pass outcome in each section. This can be sent if a state board is requiring a report with the WREB raised seal, mailed directly to them. (Texas is the exception, they require the report be mailed to the candidate.)

Certificate of Passing
What it is: This 8½" x 5½" certificate is suitable for framing. It states your name, location and year that the exam was taken and passed; no score is documented.

Dental Hygiene Summary Profile Sheet
What it is: This is a summary sheet for unsuccessful candidates only which charts each validated tissue trauma, calculus, and probe and recession error.

Dental Examination History
What it is: For candidates (currently California only) who are only required to show proof of exam completion and status of pass or fail. This information is sent directly to the Dental Board of California. Download this form here.
